Immunizations

All children must have inoculations according to the schedule prescribed by the state health department. A certified statement of an up-to-date immunization report (GA 3231 form) is required by the first day of school. If you have chosen not to give your child immunizations then you must turn in the appropriate form obtained from the Health Department.
